Newport & Central PCN started life in June 2020. We look after approximately 58,000 patients and work in collaboration with 4 surgeries:

Vision, values and ambitions

Our vision for the future of Newport & Central PCN is to create a patient care system that includes everyone. A system where member practices work closely together and in collaboration with health and social care, voluntary sectors, community groups and local people to deliver efficient and timely personalised care. To make the best use of available resources and to ensure that everyone receives the right support, in the right place, at the right time.

With a growing population increasingly living a 24/7 lifestyle, there is strong evidence that health outcomes are socially determined and that the conditions in which people are born, grow, live, work and grow old influence their health. Our vision focuses not only on direct health service provision but also on helping people to help themselves and their local communities; offering extended access to new and existing services is very important in creating our vision of care.

Ambition

Our ambition is to provide a joined-up care system where everyone has access to the care they need quickly and effectively.

Prevention

We will support people to live healthier lives, reducing their risk of developing health conditions in future. In addition, we will proactively identify those at risk of ill health and put early measures in place to ensure people live longer healthier and happier lives.

Supported Self-Management

We will work in partnership with the individual to help them to understand the nature and impact of their health condition, empowering them to have the knowledge, skills, confidence and support they need to manage their condition effectively in everyday life.

Education and Empowerment

We will provide people with the tools they need to understand when and how to access healthcare services, allowing well people to thrive and those that are struggling to be supported to ensure the whole population benefits.

New Models of Care

We will streamline our care processes and pathways, creating opportunities for new, innovative and effective models of care to be trialled and adopted.

What is a Primary Care Network?

Primary Care Networks were introduced in 2019 as part of the NHS Long Term plan.  As the population grows and people live longer, often with more complex needs,  GP services have had to adapt to these changing needs.

GP practices are working together with community, mental health, social care, pharmacy, hospital and voluntary services in local areas in groups of practices known as Primary Care Networks.

Additional Roles Reimbursement Scheme (ARRS)

Primary Care Networks have been given funding to provide additional roles to create bespoke multi-disciplinary teams within their individual network. 

PCN’s assess the needs of their local population and, working with local community services, make support available to people where it is most needed.

Clinical Director

Dr Waldendorf is a GP Partner at Linden Hall Surgery in Newport.  In addition to his work within the practice, Dr Waldendorf is also Clinical Director, this role is to bring together practices and stakeholders to collaborate for the development and delivery of services to the local population in our network. 


Emily Marston
Digital Transformation Lead

Emily’s role is to help the PCN adopt new technology and other initiatives to improve access to care and enable practice staff to wok more effectively to support the sustainability of general practice services. 


Amanda Lloyd
PCN Manager

Amanda manages the PCN ensuring that we follow the contractual guidance and meet the various targets set by NHS England and Shropshire, Telford & Wrekin ICB. Her background is in Primary Care, and has strong local knowledge, having worked in and around Shropshire for over 15 years.

Operational Lead

Lauren co-ordinates ARRS staff activities and functions.

Katherine’s role is to help support the administrative function of the PCN, providing essential support to the Senior Leadership Team within the PCN and also supporting the Home Visiting Team and Extended Access Team.
